Script supervisor daily report template
One page for the whole shoot day: scenes and pages shot, setups, takes, circled takes, running totals against the schedule, and a notes-to-editor block. Print it, fill it at wrap, and hand it to the production office.

What a daily report is
The script supervisor's daily report is the one-page summary of what the day actually shot, measured against what it was supposed to. It tells the production office whether the schedule is holding, tells the editor what coverage exists, and becomes part of the permanent record of the production. Producers read it at breakfast to know if the show is on track.
It is not the continuity sheet. The continuity sheet is take-by-take detail; the daily report is the totals that come out of it. You fill it at wrap, from the day's continuity sheets, in a few minutes.

Head the report
Production, director, script supervisor, date, shoot day, call time, first shot, and camera wrap.
Fill the day totals
For each row, log what was scheduled, what you shot today, the running total to date, and what remains.
Break it down by scene
List every scene shot today with pages, setups, takes, circled takes, and a status like complete, partial, or pickup.
Write notes to the editor
Flag missing coverage, continuity issues, wild lines, or a scene held for a reshoot.
Sign and send
Sign it, get the 1st AD signature, and send it with the day's paperwork.
When to use it
Fill one at the end of every shoot day, on any production with a schedule to hold: feature, series, commercial, or a bigger short. It travels with the day's other reports to the production office, and a copy goes to the editor so cutting can start with a clear picture of what exists. Kept day over day, these reports are how anyone reconstructs the shoot later.
